Skylink MS-200 — Warranty Registration Form

Warranty registration form for the Skylink MS-200 Self-Monitoring Station. Use this form to register for the free one-year warranty or purchase an extended warranty.

How Registration Works

Complete both parts of the form and submit within the first year of registration for a valid warranty. The first year of warranty coverage is free of charge.

Information Required

• Name, address, city, state/province, country, zip/postal code
• Contact number (day and night), email
• Date of purchase and place purchased
• Product name and model number
• Serial number — found on the back of the Self-Monitoring Station, starting with 2 letters followed by 5 digits (e.g. AA12345)

Payment Methods

Credit Card: MasterCard, Visa, or American Express — name on the form must match the name on the card.

Cheque: Mail the cheque along with the signed registration form to one of the addresses below.

Important Notes

• Proof of purchase payment is required for the warranty to be valid.
• Include a copy of proof of the purchase date.
• The form must be signed to be valid.
• Submit within 365 days after purchasing the product.
