    August 31, 2026 4 min read

Wiring, installation, and Z-Wave network inclusion instructions for the Leviton Decora Smart Z-Wave 800 Series Switch, Cat. No. ZW15S.

    Full text manual (for reference)

    Overview

The Leviton Decora Smart Z-Wave 800 Series Switch (Cat. No. ZW15S) is designed for use with modern Z-Wave hubs, letting you control lights via app commands, scheduling, and voice control once added to your Z-Wave hub of choice. It's compatible with any Z-Wave-enabled network, regardless of manufacturer, and can also act as a repeater to strengthen your Z-Wave network.

    • Do not use with metal wallplates or aluminum wire.
    • Rocker: press top to turn ON, bottom to turn OFF.
    • Status LED illuminates when lights are OFF (for finding the switch in the dark) and provides connectivity feedback.

    What You Need

    • A Z-Wave compatible hub.
    • A Z-Wave mesh network to join — install within 50 ft of the hub or another Z-Wave repeater (communication through exterior surfaces may reduce range).
    • An available interior switch wallbox with line (hot), load, neutral, and ground wires.
    • For 3-way/multi-way control: a wired DD0SR Companion device (see leviton.com/decorasmart for connection instructions).

    Wiring Instructions

WARNING: TO AVOID FIRE, SHOCK, OR DEATH, turn OFF power at the circuit breaker before wiring.

    1. Remove the wallplate and disconnect the existing switch.
    2. Restore power briefly and carefully test wires with a voltage tester to find the line (hot) wire, then turn power back OFF at the breaker.
    3. Mark the wires: neutral wires (typically white, bundled inside the box) are required to operate this device.
    4. For single-pole installation: work only one connection at a time, and connect wires using the included wire connectors (Line, Load, Neutral, Ground).
    5. Neutral connection is required. The YELLOW/RED traveler wire is used only in 3-way applications — cap it with a wire connector even when not in use.
    6. If your wallbox wiring looks different from the standard diagram, contact Leviton's technical support team or consult an electrician.

    Test & Install

    1. Restore power and confirm the switch is receiving power, then turn power back OFF at the breaker before completing installation.
    2. Gently push the wires into the wallbox and screw the switch to the box.
    3. Snap on the Decora wallplate and restore power.

    Adding to a Z-Wave Network

    SmartStart

The switch supports SmartStart — scan the Z-Wave QR code on the device (top of unit, strap, or box) with a compatible Z-Wave controller. The device is added automatically within 10 minutes of being switched on and in network range.

    Setup Mode

    1. Hold the top rocker for 7 seconds — the Status LED turns AMBER, then blinks AMBER once released. The device is now in setup mode.
    2. To exit, hold the top rocker for 7 seconds again until the LED turns AMBER; the device returns to normal operation.

Network Wide Inclusion (Z-Wave Plus certified hubs)

    1. Enter setup mode.
    2. Tap the top rocker once — the Status LED quickly flashes GREEN, and the device is ready to be learned into the network.
    3. Use the Z-Wave controller to complete enrollment.
    4. The device's Status LED turns OFF then flashes GREEN 4 times to confirm success (RED 4 times if unsuccessful).

    Classic Inclusion (older controllers)

For older controllers without network-wide inclusion, use Classic Inclusion — the device may need to be 3–35 feet from the controller during inclusion.

    1. Enter setup mode; follow the Z-Wave controller's instructions to enter inclusion mode.
    2. Tap the top rocker once — the Status LED quickly flashes GREEN.
    3. The controller confirms successful inclusion.

    Exclusion from a Z-Wave Network

    1. Enter setup mode; follow the controller's instructions to enter exclusion mode.
    2. Tap the top rocker once — the Status LED quickly flashes GREEN.
    3. The controller confirms successful exclusion.

    Customization

    Status LED Behavior

LED ON when load is ON: Enter setup mode → tap rocker 4 times (LED flashes GREEN/AMBER) → tap rocker 1 time (LED flashes GREEN 4x to confirm).
LED ON when load is OFF (default): Enter setup mode → tap rocker 4 times → tap rocker 2 times (LED flashes AMBER 4x to confirm).
LED OFF: Enter setup mode → tap rocker 4 times → tap rocker 3 times (LED flashes RED 4x to confirm).

    Factory Default Reset

Use the exclusion process as best practice for removing a device. If the network's primary controller is missing or inoperable, use this override procedure:

    1. Hold the top rocker for 14 seconds — after the first 7 seconds the LED turns AMBER; continue holding until it flashes RED/AMBER, then release.
    2. The device resets.

    Troubleshooting

    • Wired and installed but light doesn't respond: Confirm power is restored at the breaker and wiring connections are correct.
    • Power applied, light OFF, no response: Confirm line (hot), neutral, load, and ground connections; verify the white conductor is neutral.
    • Lights work but flicker: Test the light bulb with a voltage tester to confirm line and neutral are properly connected to the fixture.
    • Cleaning: Use a damp cloth with mild soap. Do not use disinfecting sprays, foggers, or other atomized cleaning agents.
