    Installation instructions for the Leviton 40215 wall-mounted power/telephone jack combo — a low-voltage power supply and CAT 3 telephone jack designed for installing cordless telephones.

    Overview

    The Leviton 40215 (Out-of-Sight Power Jack) is a wall-mounted device providing a CAT 3 telephone jack and a low-voltage regulated power supply for cordless telephones. It replaces bulky plug-in AC adapters with a clean in-wall power source, using an interchangeable output plug system and a slide switch to select the correct output voltage for the connected phone.

    Installation Overview

    Installation requires three separate connection steps: (1) Electric power connections, (2) Communication (telephone) circuit connections, and (3) Low-voltage power connections. This device must be installed by a professional installer familiar with residential wiring practices, and all wiring must comply with local, state, and federal codes.

    1. Electric power: Turn off power at the breaker/fuse and verify power is off. Strip 3/4" of insulation from each power conductor. Connect BLACK (hot) and WHITE (neutral) leads per the wiring diagram, twisting strands tightly and securing with wire connectors and electrical tape. Mount so "TOP" is oriented correctly, then restore power.
    2. Communications (telephone) wiring: Choose one of three wiring methods depending on the installation — Recommended (wire enters from outside the wall box), Alternate (for masonry/tile/brick, wire enters from inside the box, with flexible tubing separating power and phone wires per NEC Article 800), or Surface wiring (wire enters from the outside surface of the wall). A minimum 0.25 in. (6.35mm) separation must be maintained between power and communication conductors.
    3. Low-voltage power / output plug setup: Review the original phone manufacturer's adapter label to determine required output voltage and plug polarity ("Positive Tip" or "Negative Tip"), select the matching interchangeable plug, and set the voltage slide switch accordingly before connecting to the phone.

    Pins 1, 2, 7, and 8 (PR3 and PR4) are not connected.

    • Turn off power at the breaker before wiring; verify power is off before working
    • Not intended for wet environments — do not use near tubs, sinks, laundry tubs, wet basements, or pools
    • Never touch uninsulated telephone wires/terminals unless the line is disconnected — ringing voltage can exceed 100 VAC
    • Never install telephone jacks in wet locations unless specifically rated for it
    • Never install electrical devices during a lightning storm
    • Rated for telecommunication equipment up to 6W (7–9 VDC @ 660mA max, or 9.1–12 VDC @ 500mA max) with a low-voltage DC output supply only

    Warranty

    Limited 2-Year Warranty: Leviton warrants this product free of defects in materials and workmanship for two years from the purchase date to the original consumer purchaser. Leviton's sole obligation is repair or replacement of defective units when returned prepaid with proof of purchase. Excludes labor for removal/reinstallation and voids if the product is installed improperly, overloaded, misused, opened, or altered.
