Getting Started Guide for the Leviton Decora Smart Z-Wave Plus Fan Speed Controller (Cat. No. ZW4SF), a Z-Wave enabled device that lets you select one of 4 fan speeds or turn a ceiling fan off from a smartphone, tablet, or by voice with a compatible Z-Wave hub.

The Fan Speed Controller supports SmartStart — scan the Z-Wave QR code found on the top of the controller (or the back of the box) with a controller providing SmartStart inclusion. No further action is required; the product is added automatically within 10 minutes of being switched on and in network range. The QR code, DSK, and PIN code are printed on the label on the top of the unit.
Best practice is to use the exclusion command in the Z-Wave controller: hold the paddle top for 7 seconds to enter programming mode, follow the controller's exclusion directions, then tap the paddle top once.
Use only when the controller is inoperable or missing (exclusion is preferred otherwise). Hold the top of the paddle for a total of 14 seconds — first 7 seconds turns the status light amber, continuing another 7 seconds flashes it red/amber quickly. Release to reset the device.
Enter programming mode (hold paddle top 7 seconds), tap the paddle top 4 times (status light flashes green/amber quickly), then choose:
Set the max speed lower than full to prevent wobbling, or raise the minimum for larger fans that struggle to start at the lowest level.
Enter programming mode, tap the paddle top 7 times (flashes red/amber), then use the fan speed bar to set the desired turn-on speed, or select Off to always return to the last-used level. Hold the paddle top to exit.
